To request a copy of your records please fill out a Release of Information form. Be sure to sign and date the document and submit the completed form to our Release of Information Department.
For your convenience, you can submit the form via our Secure Online Portal, *email to firstname.lastname@example.org, fax to (505) 724-4401, or of course deliver it in person.
